Abstract

Drs. Garcia-Donaire, Segura and Ruilope conduct a detailed discussion concerning hypertension and the kidney. Since cardiovascular disease has become the leading cause of disabilities and death worldwide, they argue that strict control of cardiovascular disease risk factors is indispensable in order to prevent the atherosclerotic vascular process. Regression of target organ damage and gradual decline of associated clinical conditions should be achieved if early, prompt, and aggressive treatment is correctly applied. Cardiovascular and renal damage share equivalent etiological characteristics, and a number of studies have evaluated different stages of vascular disease along the cardiorenal continuum. Chronic kidney disease is considered a high cardiovascular risk pathology, which adds several comorbidities such as anemia or bone mineral disturbances to vascular damage progression. Inhibition of the renin-angiotensin system has proven to be effective in cardiorenal protection, and there is hope that newer drugs may be effective in weight loss and smoking cessation as well. The authors review recent data on the vascular continuum and discuss ongoing trials designed to evaluate a more complete inhibition of the RAS system.
